Every year or two, depending on how lazy I am. Super quick and easy with a vacuum bleeder (see princess auto)

You could wait until a problem arises and then change it if that makes more sense to you.

Looks like the one. Like most PA tools, it works fine for the occasional use, but they'll replace it without hassle when it breaks

It says 'web only' on the main page (not on the linked detail page for some reason) and from other posts on their 'web only' deals I don't think they'll give it to you in store for that price.

I had my fluid flushed and refilled last year when I added SS lines. I couldn't find another bike that had break feel as great as mine at the time, but now it's become spongy already so I'm gonna get the fluid changed again.

YMMV.
