I got mine from Dave Buckland, if you become friends with him he'll make you one. He cuts them out of anti fatigue floor mats. Foam thats about 1/2 thick.
Tony @ Bluestreak Racing sells Woodcraft seat pads but I don't know if they are bike specific.
You could cut one out yourself from a foam mat, make a paper template then trace it onto the foam then cut. Attach with some good glue.

Riders choice has gorgeous custom cut per model black foam, the nicest stuff I've ever seen, and the most I've ever paid for a hunk of foam.I think thinner is better. I don't want much grip through the seat, whats what my tank pads are for. I want to be able to move around on the seat easily.
Dave had mentioned that he made a seat for JZ that was cut out of TechSpec Snakeskin, i had thought the extra grip would impede my moving around on the bike, i forgot to ask JZ about it at Round 1. I meant to.. got busy. Oh well.
