How do i request a disclosure?

So i have finally received my court date... Jan of 2013...

Now the issue is... all i got was a sheet stating my offence, fine amount, and court date/time.

I dont see any information regarding requesting for a disclosure. How do i request one? Sorry... my first tickets + court date.
