Can anyone tell me what model and year bike this comes from? I'd appreciate it big time!!

From my expertise I'm pretty sure its a Honda

I think you're right about the model, but I think the tank is stock. Here's another modded CB500T cafe racer;

While looking at comparison photos I found a heap of fantastic looking CB500T's such as;

And this one dropped my jaw;

Think your right, thought it was more squared but I think its just the angel of the pic.

And upon further inspection, I don't think this one actually is a CB500... but that other one I found definitely is and I'm drooling over it.
