How accurate are the sizing charts for Alpinestars? (particularly jackets). I've heard Alpinestars apparel runs small.
I'm about 5'11 and ~150pounds with ~38inch chest. My measurements seem to fall between small/medium, so I'm not quite sure what size to go with. None of my local shops stock Alpinestars so I'm not able to try em on.

Would it better to go with the larger size if I plan on adding a CE back protector and/or CE chest protector?

I went with the size 40 (US sizing) Alpinestars GP-R perf leather jacket, which I guess is a medium. I would say it fits me perfectly. I've got the same measurements as you except that I'm slightly shorter than but that doesn't matter much. I don't think you need to get a slightly larger size to compensate for the additional armour. There's still enough space in the chest and back for the extra armour.
 
I've just picked up the Alloy jacket, originally in size 54. It was SUPER small, and I exchanged it for a size 58. I would say at least one size larger, closer to a medium.
 
From my experience, both their jackets and pants are on the small side. I initially ordered my Tech 1-R jacket in Euro 54, but found I could barely zip it up. Replaced it with a 56 which fits quite snugly, but that is how it's supposed to fit. Let's just say, I'll be needing to step away from the all-you-can-eat buffets if I want to be able to continue to use it this spring ;) I was smarter when ordering the pants and took the advice of Brian Van from Sportbike Track Gear and got the correct size..
